Disability Living Allowance

Disability Living Allowance is the main benefit for people aged under 65 who have problems in their day to day lives because of long term illness or disability.

It can be paid because of physical or mental health problems. The form is quite long and complex and it may be helpful to get help or assistance when completing it.

To obtain a claim form contact the Benefits Enquiry Line on 0800 88 22 00 or the Disability Contact Centre on 08457 123456 or you can download one from the Direct.gov website.

Be aware that when you request a form you will be asked questions about the help you need. Do not be put off because the questions appear to be about physical problems. Explain that you have a mental health problem and that you need a great deal of encouragement and prompting or that you need supervision to ensure your safety.
